Rib Rhinoplasty to lengthen and sharpen the nasal tip. This patient had nasal airway collapse and blockage, the correction of which is a medically necessary, substantially insured procedure which helps avoid long term adverse health consequences (high blood pressure, sleep apnea/snoring, poor exercise tolerance, weight gain).

Patient with collapse on his left nasal sidewall (red arrow) requiring a spreader graft to support the airway. He wished to have his bridge straighter on side view and the tip in better alignment. 7 months post op. His PPO insurance paid for the breathing portion of the procedure.

Nasal surgery, chin implant (from inside the mouth) and botox to slim the jaw. Restructuring the nose to clear the airway (blocked by an overgrown septum) gave a secondary benefit of a narrower tip, straightened bridge and softened profile.
Patient 29 – Rhinoplasty

3 weeks post op. Her nose now has a slightly stronger tip but still looks right at home on her face. More tip definition will appear as the swelling reduces over the next 18 months.

Patient 31 – Rhinoplasty


Description:
I strengthened her nasal tip using extended tissue from inside the nose to increase projection while simultaneously making her nose less upturned. This makes the tip stronger and narrower and prevents widening upon smiling. No direct nostril narrowing was needed. An added benefit of strengthening the internal structures of the nose (as opposed to simply laying materials on top of the existing nose) is that the nose can be made almost perfectly straight. The bridge was built up using tissues from behind the ear. When the nose needs reshaping rather than increased bulk, no rib is needed. This is a 3.5-hour operation with a 7 day recovery. No bones were broken. I call this an Organic Rhinoplasty because no synthetic materials are used.
Patient 32 – Rhinoplasty

Procedure: Rhinoplasty
The nasal tip was soft and there was a bump along the bridge. After straightening the bridge, the nasal tip was reconstructed using tissue from inside the nose to reinforce the pre-existing weak structures. The tip now is stronger so that it does not widen upon smiling.

8 years after nose, chin, and eyelid surgery. Nose surgery with silastic bridge and cartilage to tip. Chin implant. Non-incisional eyelid surgery on the left eye. Silastic implants are generally well tolerated along the bridge in Asian patients.
